Using the link below, please setup an online account. 

You will receive an email from Metabolic Code offering a link to complete the setup by personalizing your password. 

Once you have access to our Wellness Portal, you can Schedule an Appointment and complete the Patient Intake Form (wellness overview). 

You can also upload your latest test results using “Documents” in the main menu.

  • 1. Setup Your Wellness Portal

  • 2. Complete the Patient Intake Form 

  • 3. Upload the Latest Lab Test Results (if available)

  • 4. Schedule an Appointment for your first Wellness Consultation
